Welcome United Concordia Providers

As a United Concordia Companies, Inc. (United Concordia) participating network dentist, you have agreed to provide services to members enrolled under any network access arrangement.  United Concordia currently has active network access arrangements in place with the following Envolve Dental supported plans:

Effective January 1, 2021, EBO and United Concordia will enter into an agreement that will allow members of Envolve’s Ambetter Marketplace product to use United Concordia’s Advantage Plus network.  In addition, members enrolled in Envolve’s Allwell and Ascension Complete products will have access to United Concordia’s National Medicare Advantage network.

Providers can find the information on the Envolve Dental UC website at www.envolvedental.com/UC. In addition, providers can reference United Concordia leased networks in United Concordia’s Dental Reference Guide.

Yes, United Concordia providers will show up in Find a Provider (FAP).

Providers can check eligibility through the provider web portal at www.envolvedental.com/logon.

Providers can access the Electronic Funds Transfer form on this page.  Providers should follow instructions on the form and send the form and a copy of a voided check.

Providers can send an email to Envolve’s Provider Relations department when complex claims questions arise or educational assistance is requested. ProviderRelations@EnvolveHealth.com.

If the provider contracted with United Concordia is not showing in the Find a Provider, the provider should call Envolve Customer Service at the appropriate number listed below for assistance.

Representatives should direct the United Concordia provider to follow these steps:

  1. Go to UnitedConcordia.com.
  2. Sign in on the Dentists page.
  3. Click on the Schedule of Allowances tile.

The provider should contact United Concordia at 800-307-8514.

Providers can submit claims via one of the following:

1.      Envolve’s PWP at https://pwp.envolvedental.com/,
2.      Clearing House (Payer ID 46278), or
3.      Mail in paper claims.
